You’re on a roll! Just complete the patient intake forms and at the end you will be prompted to schedule your consultation appointment with our medical team.

After scheduling your appointment, please review the appointment confirmation email and follow the additional instructions to watch the ‘How To Inject‘ videos.

  1. Be sure to completely fill out the forms! Like, we really do need your first & last name. 
  2. Omitting info or required fields will cause delays. We wouldn’t want that to happen, now would we?
  3. You must upload a photo of your ID or drivers license within the Intake process. Take a photo now before you start.
  4. After you submit your intake and consent forms, click the Scheduling button to head over to our online scheduler.
  5. Not ready to schedule? Don’t lose your way! Be sure to save the link or bookmark the page.
  6. Appointment scheduled? Review your appointment confirmation email.
  7. Watch the ‘How to Inject’ videos that are required before your appointment! The nurse may give you a pop-quiz!
